This free booklet explains the process of planning for your future decision-making and your options if you need support in the future with:

  • making decisions about your finances
  • medical treatment
  • and how you would like to live.

This booklet does not contain the legal forms you will need if you wish to appoint someone to support you to make decisions. There are links to these forms at the bottom of this page.

Enduring power of attorney appointment form

The Office of the Public Advocate’s website has links to forms for making an enduring power of attorney.

Advance care planning and forms

The Health Vic website has links to forms for documenting your decisions about medical treatment in future if you are not able to communicate your wishes or if you no longer have decision making capacity.
